Supplier Agreement

Only a representative authorized to execute agreements on behalf of your company may sign the Supplier Agreement.

Please also include a copy of your Certificate of Incorporation or Business Registration.

Supplier Setup Form

The same authorized signatory that signs the Supplier Agreement must also sign the Setup Form. On this form, you must include:

• Primary contact data
• Bank information as to where funds should be electronically deposited
• E-mail addresses of key people to receive detailed remittance advices regarding what payments have been made

Please also attach one of the following to the Supplier Setup Form showing account number and account name at the bank identified:

• An original voided check
• Bank statement (if account has no checks)
• Letter from your bank on bank letterhead

Once you have completed the Supplier Agreement and the Setup Form, please both fax and mail the information to your Citi representative.
